




| Schoharie County Child Fatality Review Team The purpose of the Schoharie County Child Fatality Review Team is to: 1. Improve collection of data regarding the number and causes of child deaths in Schoharie County 2. Promote thorough investigation of child deaths 3. Share information regarding the investigation, intervention and prevention of childhood deaths 4. Promote and provide training to support a comprehensive, multidisciplinary response to childhood deaths 5. Recognize patterns in the incidence of childhood deaths 6. Identify and solve problematic systemic issues 7. Encourage cooperation and communication among agencies charged with investigation, intervention and prevention of childhood deaths The Child Fatality Review Team will review any and all cases in which a child under the age of 18 dies under suspicious or unexpected circumstances and the case falls within the jurisdiction of Schoharie County. Reviews shall be conducted quarterly. Participation on a fatality review team must include representation. 1. Coroner 2. Law Enforcement 3. Child Protective Services 4. CFRT Coordinator 5. District Attorney 6. Medical 7. Health Department 8. New York State Office of Children and Family Services 9. County Attorney 10. Others as determined by the team |
